Value-based medicine is the practice of medicine based on the quality-of-life impact of a treatment intervention. Utility values allow a quality-of-life measurement associated with a health state and are used in cost-utility analysis, which can demonstrate the cost-effectiveness (or lack thereof) of a treatment intervention. Several questionnaires have been developed to evaluate quality-of-life issues associated with adult strabismus and its treatment, and strabismus surgery appears to be a very cost-effective treatment. In an era of increasingly limited financial resources allocated for health care services, it is imperative that maximum return is obtained from investment of health care dollars. Value-based medicine will likely result in cost-efficiency and better quality of care.

Key words: strabismus, value-based medicine, QALY, cost-utility analysis
